He might have a bowel blockage. I've got a friend that has this problem with her 10 yr. old. She won't use the bathroom for a while, and then when she does her poop is so hard and big as your arm. Her breathe will smell like poop because it backs up so far, that it goes all the way up to her throat. There's some drink that she has to drink so she will keep herself flushed out and won't get impacked. Maybe a couple of teaspoons of olive oil and empsom salt in a glass of water will do the trick.
